YOUTH SUMMER CAMP DISCLAIMER FORM

Terms and Conditions

  • As custodial parent(s) or guardian(s), I request that my child(ren) be permitted to participate in the Y.A.Y.L. (Young Adult Youth Leaders, Inc.) summer camp program.
  • I understand and agree that my child(ren)’s participation in this summer camp and its activities is voluntary.
  • I understand that my child will be participating in field trips. My child has permission to take the mode of transportation provided by Y.A.Y.L. to and from field trip sites. Campers will be under the supervision of staff and volunteers at all times.
  • I agree to direct my child to cooperate and comply with all reasonable directions and instructions from adult leaders at Y.A.Y.L. summer camp, and to observe all camp rules of conduct. I understand that if my child fails to observe those rules, or otherwise becomes disruptive in camp, I will be notified, and I agree to come to the camp site to take my child home. I understand and agree that any financial obligations resulting from vandalism, stealing, or related activity by my child will be my responsibility.
  • In consideration of Y.A.Y.L. permitting my child to attend and participate in the summer camp activities, I release, forever discharge, and promise not to sue Y.A.Y.L., it's owner or any of its affiliated entities, employees, agents, and volunteers from all liability for any loss or damage, and any claim or demands therefore on account of serious or mortal injury to the body, injury to psyche, or injury to property of my child, whether caused by negligence or other conduct by Y.A.Y.L. while the child is participating in summer camp while in our care.
  • I further agree to indemnify and hold harmless Y.A.Y.L. from any loss, liability, damage, or cost it may incur due to the presence of my child while participating in summer camp, whether caused by the negligence of Y.A.Y.L. or otherwise.
  • PHOTO RELEASE: I grant the irrevocable and unrestricted right to use, reproduce, and publish photograph(s)or video(s) of my child, including his/her image and likeness for Y.A.Y.L. publications, presentations, advertising, or website(s), or any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same. I hereby release Y.A.Y.L. from any and all claims, actions, and liability of whatever nature and relating to the use of said photograph(s) and/or video(s). If this is not acceptable to you, please notify the Director in writing.

 

AUTHORIZATION FOR HEALTH CARE:

I certify that I am the custodial parent or guardian of the below referenced minor and that this child (or children) has my permission to participate in all Y.A.Y.L. summer camp activities. Should emergency medical treatment be necessary for my child and I am unable to be contacted immediately, I do hereby give permission to the delegated agents of the Y.A.Y.L., their representatives, employees, agents and assigns, including, but not limited to staff and adult supervisors at Y.A.Y.L., to act on my behalf and to authorize (1) necessary transportation of my child in connection with the emergency, (2) the release of records necessary for insurance purposes, and (3) medical or dental care for my child. I further authorize Y.A.Y.L. staff to administer medication as indicated, and relieve the Y.A.Y.L. staff of any and all liability related to its administration in accordance with the directions on the label. This authorization shall remain effective until my child completes his/her activities at summer camp or until sooner revoked by me in writing, and may be photocopied. I understand and agree that as parent/guardian, I will be responsible for any costs associated with the medical care received by my child pursuant to this authorization.


I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE, AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
